Anyone with a kidney transplant been told not to swim in lakes rivers, anything outside of a chlorinated pool?

Answer:
Nobody taking the kind of strong steroids used in any type of transplant should swim in lakes or rivers. They could easily catch leptospirosis or another infection and die.
I personally haven't had a transplant of any kind, but I assume you were told so since many lakes and rivers are quite polluted, and since you body absorbs the toxins...that go to your kidneys to be processed...this would cause an overflow of stress on you kidneys that are in a weakened state already...fighting the body for acceptance, all the anti rejection drugs etc. But thats just my own thoughts. Listen to your doctors, they want whats best for you.
